I bought the original shoe from http://www.ebay.com/usr/4everfunky. Unfortunately, the base shoe I used is no longer available, but 4everfunky has a ton of similar and cute shoes to use.
I sanded the base of the shoe (since it was made of wood) a bit to get a rough textured surface and used Apoxie Sculpt to add the details. After everything cured, sand, prime and paint. I was lucky with these since the straps were removable and could take them off for the painting.
