Now suprisingly, I easily found the right sort of striped pants, but I am having a tremendously hard time finding the right shirt. Its simple. I need a blue and white striped long sleeve shirt, and I haven't been able to find one. The stripes need to be about 2 inches in thickness, i'd say, and I just for the life of me cannot find one. You might also want to look into rugby shirts? This looks very close in color and stripe size:

The collar and end of the sleeve isn't right though, but maybe you'd be able to alter those.
