Yes, you can create those shapes and still be able to wear it. The technique I recommend is called trapunto
, it's a quilting method where two fabrics are sewn together first, then a small hole is put in the back layer and the space is stuffed.
In this case, I'd actually get two body suits, this ensures your backing fabric is just as stretchy as your outer fabric. make sure to use a stretch stitch when attaching the two together.
It can be tedious on a full body scale like this, but it will look awesome when completed
